Janice T. Smith is a board-certified Family Nurse Practitioner dedicated to providing compassionate, evidence-based primary care to individuals and families. With over 20 years of combined nursing and advanced practice experience  in primary care, internal medicine, palliative care, home care, telehealth, and chronic disease management, she partners with patients to achieve their best health through prevention, education, and personalized care.Â

Yes, we offer telehealth visits for patients who prefer the convenience of virtual care. This is ideal for follow-ups or when you're unable to travel.

Our annual wellness visits include a comprehensive physical exam, health risk assessment, and personalized recommendations for preventive care and lifestyle.
How can I reach the office in an emergency?
In case of an urgent medical emergency, please call 911 or go to the nearest emergency room. We are not equipped to handle life-threatening emergencies.
